We recently updated our Patient Agreement form to more accurately reflect Marshall Health’s policies and to assure that the wording of the form complies with applicable laws.
If you have received services from a Marshall Health provider in some capacity during the past four years, you likely have received an informational letter from us regarding this change. For your reference, we also provided a copy of the updated form. At your next Marshall Health visit, you will be asked to fill out the updated form; no action is needed until then. This update will not have any effect on any treatment provided to you by Marshall Health.
